Massachusetts Medical License Application

Start by setting up an online profile via FCVS and designate the MA board to be the recipient of your profile. Alongside this, fill out the application form for the MA license and make the necessary payment. • A supervising physician with a legitimate MA medical license.

The initial application comes with a fee of $225, and renewal is $150.

RNs must pay $120 and APRNS — $180. 15 contact hours of CME, including a course on the diagnosis, treatment, and care of patients with cognitive impairments are also required. They must also comply with good moral character, mandatory abuse reporting, and prescriber training requirements.

LPNs must pay a $60 fee and complete 15 contact hours of CME, which includes a single course on the evaluation, therapy, and treatment of patients with cognitive disorders.
