Louisiana Medical License Application

If you want to apply for a physician or physician assistant license in Louisiana, you can start the online initial application process by logging in to the LaMED DashBoard, where you will be able to submit your application, upload the documents, pay the required fees, and check your application status.

However, some of the documents should be mailed to the State Board of Medical Examiners (LSBME). These include:

  • Oath or Affirmation (must be notarized)
  • Third-Party Authorization (must be notarized)
  • Photograph Page (passport quality)
  • Statement of Legal Name
  • FCVS Application
  • Background Check

It is important to note, that LA does not accept any electronic signature or notarization. So extra shipping times will have to be factored in for processing.

Additionally, you need to submit a passport-quality photograph that is a 2×2 clear, head and shoulders photo taken within the last 6 months.

You must complete the FCVS application in order to apply for licensure, and LSBME conducts background checks as part of the application process. The fee for licensure is $275.00.

Physician assistants must be certified by the National Commission on Certification of Physician Assistants (NCCPA) to apply for licensure and must request the initial PANCE score report to be sent directly to LSBME.

Louisiana CDS License

To apply for a CDS license, you must provide:

  • Valid Louisiana medical license
  • Valid DEA registration
  • Copy of your driver’s license or state ID
  • Copy of your social security card
  • Copy of your birth certificate or passport
  • Copy of your diploma or transcript from an accredited medical school

You must also complete at least 3 hours of Board-approved continuing medical education (CME) that includes all 4 of the following topics: best practices for the prescribing of CDS, drug diversion training, appropriate treatment for addiction, and the treatment of chronic pain.

Louisiana State Board of Nursing License

To obtain a nursing license, you must meet certain requirements: graduate from an approved nursing program and pass either the NCLEX-RN or the NCE-APRN.

The fee is $100 for RNs or $140 for APRNs, and the timeframe may vary based on individual circumstances and factors beyond the control of the Board.

Louisiana Medical Board License Renewal

To ensure that your license remains valid, you must go through the Medical Board License Renewal process every two years.

One of the key requirements is completing continuing education courses. Physicians are required at least 20 hours of CME credits per year. Physician assistants and nurses, on the other hand, need to complete no less than 15 hours per year.

Another crucial requirement for license renewal is the completion of an LSBME-approved Controlled Dangerous Substances (CDS) Renewal course. Physicians are required to complete it once every three years, while physician assistants and nurses — once every two years.

Louisiana Telemedicine License

Out-of-state physicians who want to practice telemedicine must obtain a telemedicine permit issued by the Medical Board. It requires possessing the qualifications for licensing prescribed by the board and holding an unrestricted license issued by another state. The fee is $300, which is not refundable. The renewal fee is $200.

Physicians must establish a bona fide physician-patient relationship before examining a patient. It’s important to note that the permit issued by the board doesn’t allow them to open an office, meet with patients in person, or receive calls from patients in Louisiana.
